Some insurance companies require authorization prior to your first visit; which our office performs for you. Presently we accept most insurance plans, but benefits vary based on whether we are in or out of your network. If you are unsure whether Suburban Physical Therapy is in your network, contact your insurance company directly.

Self-Pay
